Transitions between hospital, community, and home care are times of increased risk for medicine errors and miscommunication. Studies show that between 30% and 70% of patients experience an error or unintentional change in their medicines when moving between care providers.
Our Discharge Medicines Review (DMR) Service helps keep patients safe during these transitions. Working closely with hospitals, GPs, and carers, our pharmacists ensure that any medication changes are clearly understood and managed — reducing confusion and preventing potential harm.
The Royal Pharmaceutical Society (RPS) and other professional bodies highlight the importance of strong communication between healthcare providers. By choosing to register with our pharmacy for this service, you’re ensuring a smoother, safer recovery after hospital discharge.
